You have Keratoconus. Now what?

Once you are done asking the question, WHY ME? it is time to address the condition and move on with your life.

Keratoconus, Pellucid Marginal Degeneration, Keratoglobus, Irregular Astigmatism, and post-LASIK Ectasia as well as Forme Fruste variants of these conditions are more common than you think. Fortunately, excellent treatment exists and new modalities finally achieved FDA approval in 2016.

The most effective approach to keratoconus treatment, or more properly phrased, ectasia management, is highly individualized. Experienced and creative exploration of techniques from minimally invasive, to more aggressive and definitive care is the fastest path to a solution.

We have a full spectrum of diagnostics including tear film assessment, topography and wavefront analysis, confocal microscopy, optical coherence tomography (OCT), and HD video imaging. Surgical therapeutic options include collagen crosslinking* (CXL or KXL), Intacs, Intralase (femtosecond laser tunneling), DALK (Deep Anterior Lamellar Keratoplasty), MDALK (Maximal DALK, Big Bubble), Full Thickness Keratoplasty (Corneal Transplant), and Excimer Laser post-keratoplasty Corneal Resculpting. Additionally, refractive options include scleral contact lenses, rigid gas-permeable contact lenses, custom soft contact lenses, and Visian ICL (and Toric ICL).

The literature claims over 50% of corneal transplant patients still require RGP (rigid gas-permeable) contact lenses after surgery. Our practice has less than 10% who still require any kind of contact lens. This is the result of ERAPK, Advanced Surface Ablation for Excessive Regular Astigmatism following Penetrating keratoplasty, a concept Dr. Lewis submitted to the FDA over a decade ago. Our practice enjoys the largest series of ERAPK treatments worldwide.

Patients with early or forme fruste keratoconus can, and probably should, be evaluated for corneal collagen crosslinking. If appropriate, we believe this will halt and, in some cases, reverse much of the visual degradation.
